Artesunate for Severe Malaria Treatment

Artesunate is a medication specifically designed to treat severe malaria in both adult and pediatric patients. It is the first-line treatment recommended by the World Health Organization for severe malaria cases. Artesunate works by rapidly converting into its active form, dihydroartemisinin, which then targets and destroys the malaria parasites. This process involves the generation of reactive oxygen species that increase oxidative stress and damage malarial proteins, ultimately leading to the parasite’s destruction.

Artesunate is preferred over quinine for severe malaria treatment due to its higher efficacy in preventing deaths. It is often used in combination with other antimalarial drugs, such as mefloquine, to enhance its effectiveness. The medication can be administered intravenously, intramuscularly, orally, or rectally, depending on the severity of the condition and the patient’s tolerance.

Artesunate is effective against Plasmodium falciparum, Plasmodium ovale, Plasmodium malariae, and severe Plasmodium knowlesi infections. However, it is not recommended for the treatment of Plasmodium vivax infections due to high resistance rates. The medication is available in various formulations and strengths, making it suitable for different age groups and treatment scenarios.

When using artesunate, it is crucial to follow the prescribed dosage and administration instructions carefully. The typical treatment duration is three days, with the first dose of sulfadoxine/pyrimethamine administered alongside the initial dose of artesunate. It is essential to note that artesunate should not be used in patients with allergies to sulfonamides and should not be combined with co-trimoxazole. Additionally, folic acid should not be given on the same day as sulfadoxine/pyrimethamine or within two weeks thereafter.
