Dobutamine is a medication used to support the heart in patients with cardiac decompensation due to depressed contractility. It is particularly useful in cases of cardiogenic shock characterized by severe heart failure and hypotension, as well as septic shock where it may be added to dopamine for improved ventricular function and reduced filling pressures.
Each ampoule contains 250 mg of dobutamine hydrochloride, which is equivalent to 50 ml of solution. The concentration is 5 mg per milliliter. This clear, colorless or slightly yellow solution is administered intravenously.
It is important to note that dobutamine contains 3.06 mg of sodium per milliliter, which translates to 153 mg in a 50 ml ampoule. This should be considered for patients with sodium restrictions.
Dobutamine works by acting as a selective ?1-adrenergic agonist, increasing the strength of heart muscle contractions. It is rapidly metabolized by the body, with most of its inactive metabolites excreted in the urine within the first two hours. The half-life of dobutamine in plasma is approximately two minutes.
For laboratory use only, dobutamine hydrochloride is sparingly soluble in water and ethanol, forming a clear solution with gentle heating. It is sensitive to light and should be stored at 2-8 °C to maintain its stability.
