Colitran DS: A Comprehensive Antibiotic Solution for Multidrug-Resistant Infections.

Colitran DS is a powerful antibiotic medication designed to combat multidrug-resistant Gram-negative infections. This last-resort treatment is particularly effective against bacteria such as Pseudomonas aeruginosa, Klebsiella pneumoniae, and Acinetobacter. It comes in two forms: colistimethate sodium, which can be injected intravenously or intramuscularly, and colistin sulfate, which is applied topically or taken orally.

Colistimethate sodium is a prodrug that undergoes hydrolysis to form partially sulfomethylated derivatives and colistin. This process makes it less toxic when administered parenterally. The dosing for Colitran DS can be complex, with different labeling for parenteral products in various regions. For example, Colomycin 1,000,000 units is equivalent to 80 mg of colistimethate, while Coly-Mycin M 150 mg of colistin base is equivalent to 360 mg of colistimethate or 4,500,000 units.

Colistin works by interacting with the bacterial outer membrane, displacing magnesium and calcium ions, and disrupting the cell membrane. This mechanism leads to the leakage of intracellular contents and ultimately results in bacterial death. Colitran DS is indicated for the treatment of serious infections in adults and children, including neonates, and is also used to manage chronic pulmonary infections in patients with cystic fibrosis.

When administered intravenously, Colitran DS should be given in two divided doses, with the second dose administered after the initial loading dose. The dosage can be adjusted based on the severity of the infection and clinical response. It is essential to monitor renal function, as the drug is dialyzable through conventional haemodialysis and continuous venovenous haemo(dia)filtration (CVVHF, CVVHDF).
